How to Access Your Health Care Reports and Medical Records

We have all seen the news reports about the big health insurance companies getting hacked which has exposed millions of American to identity theft and fraud.

You should monitor your health care reports and medical records.

You are entitled to a free copy of your medical reports. You are also entitled to a copy of your medical records. Depending on the doctor or hospital you may be charged a fee if you want printed copies of your records.

Medical Reports

Health care and life insurance companies use your medical reports to determine your risk factor when deciding to provide you with service.

They get the medical reports mainly from three agencies, ExamOne, Milliman IntelliScript and MIB Group.

You should request a copy of your medical reports and check the reports for any errors such as medical services listed on the reports that you never received which may be a sign of identity theft or fraud.

To request a copy of your health report call them at 1-877-933-9261 or request a copy online.  Fill out the online form and ask for a copy of your health report. If they have a report on file, they will provide you with a copy.

Milliman IntelliScript

If you have approved the release of your medical information to an insurance company and the insurance company requested a report from Milliman IntelliScript, they may have your prescription information on file. To request a copy of your report, fill out their online request form.

You will need to provide

Your first and last name, your mailing address, your date of birth, the last 4 digits of your Social Security Number and an email address if you want the report emailed to you.

MIB Group

MIB Group can provide you with your Underwriting Services Consumer File. They would have a file on you if you applied for individually underwritten life or health insurance in the last 7 years.

From Your Health Care Provider

Many doctors’ offices and hospitals offer online access to your medical records through a patient portal. Check with your doctor and hospital to find out if they offer this service.

They will provide you with instructions on how to sign up and log into your account.

Most offer access to test results, prescription information and refills, access to your medical records, appointment scheduling, bill pay and other services.

If they do not offer an online portal you can request printed copies of your medical records.

The Health Insurance Portability and Accounting Act (HIPAA) established in 1996 states that patients have the right to obtain copies of their health records. This can include medical records, doctor’s notes, test results, lab reports and billing information.
